Once you have made an appointment via phone or e-mail, you will receive forms and paperwork to fill out through a link to your e-mail. Once you have read, signed, and filled out the paperwork, you will populate in our system (SimplePractice). You will then get notifications sent to your e-mail and/or phone reminding you of your upcoming appointment.
